Academic/Administrative Petition
Complete this form if you are requesting to exceed the course limit, request an extension, or to
waive academic or administrative regulations.

Business Office Petition
Complete this form if you are requesting an exception to the refund policy, fees charged, deadline
obligations, the standard payment plan duration, or to the registration with no past due balance.

Change of Address
Complete this form if you are a current student or Alumni and any or all of your contact information
needs to be updated.

Incomplete Course Contract
Complete this form if you would like to receive an Incomplete in a particular course.

Intent to Graduate
  • If you intend on graduating within the 2013-2014 academic year this form should be submitted to the Registration office by December 6, 2013.

  • If you intend on participating in the commencement ceremony you will be billed the $150 Graduation Fee which includes but is not limited to a diploma, an official transcript and BBS Program Certification (if applicable).

  • If you do not intend on participating in the commencement ceremony you will be billed the $75 diploma fee. You will need to request and pay for any transcripts that you might need or want.


Leave of Absence Request
  • Complete this form if you would like to request a Leave of Absence
  • Students may take one (1) Leave of Absence for a maximum of one (1) year
  • A leave cannot be granted to a student with any of the following:
  • Incomplete grades
  • For more information please refer to the catalog.

Name Change
  • Complete this form if you are a current student and have recently had a name change.
  • Supporting documents required.
  • Alumni records names cannot be changed but we can add a preferred name to your records.

Program Modification
  • Complete this form if you would like to Add or Drop a course, change class day, change programs, or add a concentration.


Request for Diploma Replacement
  • Diploma replacement cost is $70
  • Complete this form if you need to replace your diploma
  • Supporting document required if there is a name change.
  • Please note that it will say “Re-Issued (Date) “
  • Processing time is 6 to 8 weeks
  • Notified via e-mail as soon as it is available for pick up. If you are unable to pick up it can be mailed via USPS Express Mail for an additional $25 within the United States. Outside the U.S. it is an additional $40.

Student Verification
  • Complete this form if you need your enrollment or degree verified.
  • Enrollment & Graduation information can be verified though the National Student Clearinghouse.


Transcript Request
  • Alumni who graduated before May 1984 will need to contact Azusa Pacific University http://www.apu.edu/alumni/transcript/ or call 1-626-815-6000 to order transcripts.

  • Current student and alumni that graduate after May 1984 should only need to requests transcripts from Phillips but depending on your graduation date, it is possible that you might need to request transcripts from both Azusa Pacific and Phillips. Phillips Registration Office will let you know if you also need to request from Azusa.
     

  • Official Transcript Processing Time and Cost:

    • Processing time is the time it takes our office to prepare your transcript. This does not include mailing time. Please note: We cannot guarantee your transcripts arrival or the time it will take to reach its destination once it has left our campus.

    • Expedited

      • Processing time is 2 business days

      • Cost is $20 for the first copy & $5 for each additional copy

    • Standard

      • Processing time is 10 business days

      • Cost is $10 for the first copy and $5 for each additional copy

    • U.S. First Class Mail

      • No additional charge

    • U.S. Postal Service Express Mail

      • $25 additional charge if mailed within the United States

      • $40 additional charge if mailed outside the United States

  • Transcript Payment:

    • Cash, Charge, Check, Money Order

    • PayPal Payment accepted when requesting online

  • Unofficial transcripts can be picked up from the Registration Office or e-mailed directly to the e-mail address that we have on record.

  • For security purposes, we do not fax transcripts.
